Hip Replacement
Revision Hip Replacement in Surat
Revision hip replacement at Vedaant Orthopaedic Center, Surat — for a hip replacement that has loosened, worn, dislocated repeatedly or become infected.
In short: Revision hip replacement removes failed components and replaces them with new ones, often using longer stems, larger cups or bone graft to compensate for lost bone. The plan depends entirely on why the original hip failed, and establishing that cause is the most important part of the assessment.
Why a hip replacement fails
- Aseptic loosening. The bond between implant and bone gives way without infection. The commonest long-term reason for revision.
- Bearing wear and osteolysis. Particles released as the bearing wears provoke a reaction that dissolves bone around the implant. Often silent until significant bone has already been lost, which is the argument for follow-up X-rays even when the hip feels fine.
- Recurrent dislocation. A hip that comes out repeatedly, from component position, soft-tissue laxity or muscle weakness.
- Infection. Early after surgery, or later from bacteria carried in the bloodstream from a dental or urinary source.
- Periprosthetic fracture. A break in the femur around the stem, usually after a fall.
- Stem or cup failure. Uncommon, but implants can fracture.
Symptoms that suggest a problem
- Thigh or groin pain returning after a comfortable period
- Pain on starting to walk that eases, then returns with distance
- A sense of the hip clunking, catching, or giving way
- The hip dislocating, particularly more than once
- A leg that feels shorter than it did
- Fever, wound discharge or a hip that becomes hot and swollen — urgent
How the cause is established
- History, including when the hip changed and any illness or dental work around that time
- Examination for leg length, stability, gait and muscle strength
- Serial X-rays compared with films taken just after the original surgery — loosening shows as change over time
- Blood tests for inflammatory markers
- Aspiration of the joint where infection is possible, with fluid sent for culture
- CT where bone loss needs quantifying before the operation is planned
Infection must be excluded first. An infected hip and a worn hip can feel similar to the patient but are treated on entirely different pathways.

What revision surgery involves
Removing well-fixed components without destroying the bone around them is a significant part of the operation and sometimes the hardest part. What follows depends on what is found:
- Where bone stock is good, a straightforward exchange of components may be enough.
- Where the femur has lost bone, longer stems bypass the deficient section and gain fixation further down.
- Where the socket has eroded, larger cups, augments or bone graft rebuild the support the implant needs.
- Where dislocation is the problem, component position is corrected, and larger heads or constrained liners may be used to improve stability.
- Where infection is confirmed, treatment is usually staged — components removed, an antibiotic spacer placed, antibiotics given for a period, and a new hip implanted only once the infection has cleared.
Revision hip surgery takes longer than the original operation, involves more blood loss, and has a longer recovery. Weight-bearing may be restricted afterwards depending on what was done to the bone.
Risks
The risks of primary hip replacement all apply, generally at higher rates: infection, dislocation, clots, fracture during component removal, nerve injury and leg length difference. Bone quality is usually worse than at the first operation, which is what drives most of that increase. These are discussed individually before surgery.
Frequently asked questions
How do I know my hip replacement is failing?
Returning groin or thigh pain, new clunking, a shortening leg, or repeated dislocation all warrant assessment. Fever, wound discharge or a hot swollen hip needs urgent review because it suggests infection.
My hip does not hurt but my X-ray has changed. Does that need surgery?
Sometimes. Bone loss from bearing wear can be painless until it is advanced, and revising before bone loss becomes severe is technically easier and gives a better result. This is judged on serial X-rays rather than symptoms alone.
Why does the hip keep dislocating?
Usually component position, soft-tissue laxity, weak abductor muscles, or a combination. Repeated dislocation is itself an indication for revision, since each episode stretches the soft tissues further.
How is an infected hip replacement treated?
Most often in two stages: the implant is removed, an antibiotic-loaded spacer placed, antibiotics given for a period, and a new hip implanted once the infection has cleared. This takes months rather than weeks.
Is the result as good as the first replacement?
Revision reliably improves pain and stability, but function does not always match a well-performing first replacement, particularly where bone or muscle has been lost. An honest expectation is a stable, comfortable hip.
How long is recovery?
Longer than after a primary replacement, and weight-bearing may be restricted for a period if bone graft or a long stem was used. The rehabilitation plan is set according to what was done during surgery.
